Through the contact form: name, email address, optional phone number, subject and message. Through the course waiting list: first name, email address, profession, chosen course and, if you give it, the priority you indicate.

No health data should be sent via these forms; medical documents are brought to the consultation.

Purposes and legal basis

To respond to your enquiry and to inform you of the launch of the training. Processing is based on your consent, obtained via the checkbox on each form.

Retention period

No more than three years after the last exchange for contact enquiries. Until you unsubscribe for the waiting list.

Recipients

Daniel Bontems and, for consultations, his colleagues at the practice. The data is processed by technical service providers (hosting, email delivery) bound by confidentiality obligations. It is neither sold nor transferred.

Audience measurement

Anonymous visit statistics are produced with Umami, without cookies and without storing your IP address. They contain neither your name nor your email address. Details on the Cookies page.

Booking appointments

Appointments are booked on Doctolib, which processes your data in accordance with its own privacy policy.

Your rights

You have the right of access, rectification, erasure, objection, restriction and portability, and you may withdraw your consent at any time, by writing to the practice via the contact form or by calling +33 6 30 81 50 89. You may also lodge a complaint with the CNIL, the French data protection authority (cnil.fr).
